The five-step support journey
Initial referral & assessment
Everything starts with a thorough needs assessment – drawing on the individual themselves, their family, social workers and health professionals — so we have a full picture before support begins.
Person-centred support planning
Together with the service user, we shape a detailed support plan that reflects their aspirations, addresses real-world needs and sets clear, measurable goals.
Matching with the right support worker
We match each service user with a compatible, trained support worker — weighing personality, shared interests and specific experience with mental health, learning disability or autism.
Ongoing review & adaptation
Support needs shift over time. We hold regular review meetings with service users and stakeholders, adjusting the plan as goals, progress or circumstances change.
Transition planning
When a service user is ready to move toward greater independence, we plan the transition carefully — making sure the next step is safe, supported and confident.
